This form needs to be filled out by undergraduate students before they
can register for a variable unit course. Students need to complete their
portion, the instructor does their portion, the chairperson signs it, and
the form is to be given to Carol Nickles and she will give you the CRN number to register.

Form is to be filled out by instructor and returned to Carol Nickles. She
will secure the chair's signature.
Please complete and submit form to Department Office prior to first day of
instruction. Form must be completed prior to deadline for adding courses
(10th day of instructor).
