The reporting of a work-related injury or illness by an employee to his or her supervisor triggers a cascade of events. The employee and supervisor must fill out forms that record the facts surrounding the event and then an investigation must be conducted by UCD Risk Management before any Workers' Compensation benefits can be awarded. Following the investigation, steps to prevent a recurrence of the injury or illness must be taken.
In the event that an employee suffers a work-related injury or illness:
- For minor injuries, apply first aid and seek additional care as needed; for more serious injuries, call 9-1-1 to request emergency assistance. Do not move a seriously injured person who is not in immediate danger from fire or other hazard.
- For non-emergency injuries or illnesses that are sustained during normal work hours [ 8:00 AM - 5:00 PM, Monday - Friday ], all employees must seek medical treatment from Employee Health Services, 501 Oak Avenue [ corner of Oak Ave and Russell Blvd; phone 752-2330 ]. For work-related injuries or illnesses sustained at other times, employees should seek treatment at Sutter Davis Hospital, 2000 Sutter Place [ corner of Covell Blvd and Sutter Place, near Hwy 113; phone 756-6440 ], or visit the nearest emergency room.
- Employees who want to be treated by their personal physician [ or other specific physician ] in the event of a work-related injury or illness, must submit a Designation of Physician form prior to sustaining an injury or illness.
- Volunteers are covered under Workers' Compensation as long as they have submitted a Volunteer/Without Salary (WOS) Appointment form.
- Students not employed by the University should seek treatment from the Cowell Student Health Center, 752-2300.
- After receiving medical treatment, the employee must report the injury or illness to his or her supervisor within 24 hours and then submit an Employee's Claim for Workers' Compensation Benefits form. An injury sustained on Friday afternoon may be reported on Monday.
- Within 24 hours after being notified by an employee of a work-related injury or illness, the supervisor must notify the Benefits and Risk Management Office by phone (752-1774) and then must submit a UCD Employer's Report of Occupational Injury or Illness form.
